Technique 07 · Crisis Self-Check

Panic attack or heart attack? Tell in 10 seconds.

The overlap is cruel — racing heart, chest tightness, can't breathe, certainty something is wrong. A 10-second mechanical self-check won't replace a doctor, but it will tell you which pattern you're in right now: the one that peaks and fades, or the one that needs a call.

Panic vs Heart

  1. Timing — does it peak and fade, or build slowly?Panic peaks within 10 minutes, then fades. Cardiac symptoms build slowly and don't fade on their own.
  2. Pain quality — sharp/moving, or heavy/centered?Panic = sharp, moves around, often better with movement. Cardiac = heavy, crushing, centered, worse with exertion.
  3. Tingling — hands and face, or arm and jaw?Panic = tingling hands/face from fast breathing, resolves with slow breath. Cardiac = pain radiating to left arm or jaw.
  4. What else? — racing thoughts, or cold sweat + nausea?Panic = vivid fear of dying, racing thoughts. Cardiac = sudden cold sweat, nausea, sense something is genuinely wrong.

Mechanical de-escalation

  1. Feet flat. Press down.Pressure into the ground. Anchor.
  2. Breathe out longer than in. 4 in, 8 out.The long exhale is the switch. Body was holding its breath.
  3. Name 5 things you can see. Out loud if you can.Brings prefrontal cortex back online.
  4. Remind yourself: this peaks in 10 minutes.Panic is self-limiting. Your body cannot sustain this for long. You are not in danger from the panic itself.
